Current opening: Care Coordinator, LPN or RN/BN working 24 – 32 hours per week
This position will act as a coordinator and liaison between the client, caregiver team and a variety of professional services based on the client’s needs, maintaining frequent, clear communication with all parties involved. The care coordinator builds compassionate relationships with families to assist them in determining the best course of action in living a full life.
Responsibilities:
• Conduct client assessments to establish person directed care plan
• Frequent, clear communication with clients and caregiver teams
• Oversee client care focused on quality of life
• Electronic documentation and delegation of tasks
• Participate in core team meetings
• On call duties

Qualifications
Qualifications:
• Graduate of a post-secondary nursing program (BN, RN or LPN)
• Active registration with the College of Registered Nurses of Manitoba/College of Registered Psychiatric Nurses
• Previous experience with Palliative Care considered an asset
• Exceptional communication skills
• Compassionate heart to serve others
• Efficient with technology
